Bee Holistic Wellness Center is an integrative naturopathic medical practice dedicated to providing personalized healthcare that supports long term wellness through thoughtful, evidence informed recommendations.

Our approach considers the whole person rather than focusing solely on individual symptoms. Recommendations are based upon each patient's health history, clinical findings, personal goals, and professional clinical judgment.

Services may include nutrition and lifestyle counseling, laboratory interpretation, specialty laboratory testing, dietary supplement recommendations, wellness education, and other therapies permitted within the legal scope of naturopathic medicine.

Bee Holistic Wellness Center is owned and operated by a licensed naturopathic doctor practicing in accordance with the laws and regulations governing naturopathic medicine in the State of Wisconsin.

Bee Holistic Wellness Center provides naturopathic healthcare services within the legal scope of practice established under applicable Wisconsin law.

Our practice is not a primary care clinic and does not provide emergency or urgent medical services. Patients are encouraged to maintain an ongoing relationship with a primary care provider for preventive healthcare, acute illnesses, chronic disease management, routine screenings, and referrals to specialists when appropriate.

Recommendations provided by Bee Holistic Wellness Center are intended to support your overall health and are not intended to replace appropriate medical care from your primary healthcare provider or other qualified healthcare professionals.

Nothing contained on this website shall be interpreted as a guarantee that any particular treatment, supplement, laboratory test, therapy, or healthcare outcome will be recommended or achieved.

Provider Patient Relationship

Accessing this website, requesting information, submitting a contact form, subscribing to communications, or scheduling an appointment does not, by itself, establish a provider patient relationship.

A provider patient relationship is established only after:

• Required intake forms and consents have been completed.

• Bee Holistic Wellness Center has agreed to provide care.

• The provider has determined that the requested services are appropriate and fall within the provider's legal scope of practice.

• Care has begun through a scheduled appointment.

Bee Holistic Wellness Center reserves the right to decline, postpone, or discontinue services when doing so is clinically appropriate, legally required, outside the provider's scope of practice, or otherwise in the best interest of patient care.

Telehealth Services

Bee Holistic Wellness Center currently provides care through secure telehealth appointments using Practice Better, our electronic health record, patient portal, and telehealth platform.

Telehealth allows patients to receive naturopathic care remotely using secure communication technology. While many healthcare concerns can be appropriately addressed through telehealth, certain conditions may require an in person evaluation, emergency medical care, or referral to another healthcare provider.

At the time of each appointment, patients must be physically located within the State of Wisconsin, where the provider is licensed to practice naturopathic medicine. Appointments cannot be conducted if the patient is located outside Wisconsin at the time of the scheduled visit.

Patients are responsible for participating in appointments from a private location using a reliable internet connection and compatible technology. Although reasonable efforts are made to provide uninterrupted telehealth services, technical difficulties may occasionally occur. Bee Holistic Wellness Center is not responsible for interruptions caused by internet service providers, equipment failures, software malfunctions, or other circumstances beyond our reasonable control. When feasible, reasonable efforts will be made to reconnect or reschedule the appointment.

Emergency Medical Care

Bee Holistic Wellness Center does not provide emergency medical services or urgent medical care.

If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, signs of stroke, severe allergic reactions, loss of consciousness, suicidal thoughts, or any other life threatening condition, call 911 or seek immediate medical attention.

Electronic communications, including email, voicemail, text messages, contact forms, and the patient portal, are not continuously monitored and should never be used to report urgent or emergency medical concerns.

Treatment Recommendations & Results

Healthcare outcomes vary from person to person. Individual responses to treatment depend upon numerous factors, including medical history, underlying health conditions, genetics, medications, nutrition, lifestyle, environmental factors, and adherence to recommendations.

Recommendations provided by Bee Holistic Wellness Center are based upon professional clinical judgment and may include nutrition and lifestyle counseling, dietary supplements, specialty laboratory testing, wellness education, and other therapies permitted within the provider's legal scope of practice.

No representation, warranty, or guarantee is made regarding any specific healthcare outcome, symptom improvement, laboratory result, or response to treatment. Patients acknowledge that healthcare outcomes cannot be predicted or guaranteed.

Statements regarding dietary supplements or wellness products have not been evaluated by the United States Food and Drug Administration and are not intended to diagnose, treat, cure, or prevent any disease.

Appointments & Scheduling

Appointments may be scheduled through our online scheduling system or other approved scheduling methods.

Scheduling an appointment does not guarantee that Bee Holistic Wellness Center will establish an ongoing provider patient relationship. Prior to providing care, we reserve the right to determine whether the requested services are appropriate, within the provider's legal scope of practice, and in the best interest of patient care.

Appointment availability, services, fees, and scheduling policies are subject to change without prior notice.

While we make every reasonable effort to honor scheduled appointment times, circumstances beyond our control may occasionally require appointments to be rescheduled or canceled. If this occurs, reasonable efforts will be made to notify affected patients as soon as possible.

Fees & Payment

Bee Holistic Wellness Center is a direct pay practice unless otherwise stated.

Payment is due at the time services are provided unless alternative arrangements have been approved in advance.

Patients are responsible for understanding the fees associated with their care before services are rendered. Current fees are available on our website and may be updated periodically without prior notice.

If laboratory testing, dietary supplements, educational resources, or other products or services are purchased through third party companies, those purchases are separate from Bee Holistic Wellness Center and remain subject to the pricing, policies, and terms of those individual companies.

A superbill may be provided upon request for patients who wish to submit claims to their insurance carrier. Bee Holistic Wellness Center does not bill insurance and cannot guarantee reimbursement for any services provided. Coverage decisions are made solely by your insurance company.

Unless otherwise required by law, fees for services already rendered are nonrefundable.

Payment Authorization

To simplify billing and ensure timely payment, patients may be asked to maintain a valid credit or debit card on file.

By providing payment information, you authorize Bee Holistic Wellness Center to charge the card on file for scheduled appointments, applicable cancellation or no show fees, outstanding balances, and other authorized charges related to services you have received.

Patients are responsible for maintaining accurate and current payment information. If a payment cannot be processed, future appointments may be delayed or canceled until the account has been brought current.

Stored payment information is processed through secure third party payment providers. Bee Holistic Wellness Center does not store complete credit card information on its own servers.

Cancellation Policy

Appointment times are reserved specifically for each patient.

If you need to cancel or reschedule your appointment, we request at least 24 business hours' notice whenever possible.

Appointments canceled without sufficient notice or missed without prior communication may be subject to a cancellation or no show fee.

Repeated missed appointments, late cancellations, or repeated failure to attend scheduled appointments may result in prepayment requirements, limitations on future scheduling, or dismissal from the practice at the provider's discretion.

Bee Holistic Wellness Center reserves the right to waive cancellation fees when circumstances warrant, at the provider's sole discretion.

Patient Responsibilities

Successful healthcare depends upon open communication and collaboration between the patient and provider.

By receiving services through Bee Holistic Wellness Center, patients agree to:

• Provide complete, accurate, and current health information.

• Inform the provider of changes to medications, supplements, allergies, diagnoses, and medical history.

• Ask questions regarding any recommendation or treatment plan that is not understood.

• Notify the provider of unexpected reactions, significant changes in health, or new medical diagnoses.

• Maintain an ongoing relationship with a primary care provider when appropriate.

• Treat providers, staff, and others with courtesy, honesty, and respect.

Bee Holistic Wellness Center is committed to maintaining a safe, respectful, and professional environment. We reserve the right to refuse or discontinue services when abusive, threatening, discriminatory, harassing, fraudulent, or otherwise inappropriate behavior occurs toward the provider or staff.

Communication

We strive to respond to patient messages as promptly as possible during normal business hours. Response times may vary based on weekends, holidays, clinic volume, or other circumstances.

Electronic communication, including email, the patient portal, voicemail, and text messaging, should not be used for urgent or emergency medical concerns. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.

Although reasonable safeguards are used to protect electronic communications, no method of transmitting information electronically can be guaranteed to be completely secure. By choosing to communicate electronically, you acknowledge and accept these inherent risks.

Privacy

Bee Holistic Wellness Center respects your privacy and is committed to protecting the personal information you share with us.

When you interact with our website, request an appointment, complete intake forms, communicate with our practice, or receive healthcare services, we may collect information reasonably necessary to provide care, operate our practice, communicate with you, process payments, and comply with applicable legal and professional obligations.
